Advocacy and support for adults in rehab or long term care facilities
Whether in short term rehab or residing in long term care, we monitor for level-of-care changes and assist in discharge planning. We can assist in understanding and appealing non-coverage notifications.
Point of contact for
family, loved ones
We are your eyes and ears monitoring for change in cognitive , physical and emotional wellness.
Transition and move coordination
Preparing for long term care, downsizing, planning moves, coordinating estate sales or disposal of discarded property, sale of real estate, petcare set up, rehoming of pets.

Step 1

Comprehensive Assessment

Includes evaluation of the person, their needs and current environment.

Step 2

Care Planning or Services Coordination Plan

Determination of current and anticipated support which may include: Transition coordination, recommendations for living at home safely (Aging In Place), discharge planning/advocacy, connection with community resources, forecasting financial needs anticipating aging related changes.

Step 3

Ongoing Care Management and/or Support

Delivering or arranging for the provision of care needed.

Why Age Advocate?

They say “Growing old isn’t for sissies,” and there is a lot of truth to that! Jennifer Harriss, founder of Age Advocate, has seen firsthand, through her public and private work in eldercare, just how challenging the aging process in today’s environment can be. From navigating the many options and resources available as well as the inherent bureaucracy in “the system,” she is committed to helping with the journey. Her deep respect for her clients, and integrity in assuring their best interests are met are hallmarks of Age Advocates’ services.

Meet Jennifer Harriss, owner of Age Advocate.  Jennifer has a passion for geriatric care. Her experience and qualifications include the following:

  • Masters of Science Gerontology, (University of North Carolina Greensboro)

  • NC Real Estate Broker for 20+ Years

  • Bachelor’s degree in Recreation, (Kent State University)

  • Former NC Department of Health and Human Service certified Regional Long Term Care Ombudsman

  • A Matter of Balance coach

  • Adult Children of Aging Parents (ACAP) leadership team Guilford County

  • Alzheimer’s Association certified family support group facilitator

  • Volunteer with Senior resources of Guilford as a Senior Health Insurance Information Program (SHIIP counselor), Nutrition Advisory Council and Meals on Wheels

  • Fortunate to be a mother, sister, long distance daughter and local daughter-in-law
